This position is part of a multidisciplinary team that provides field based psychosocial rehabilitative services to adult individuals with severe and persistent mental illness. Other services and coordination of services include: medication training and supports, supported housing and employment services, co-occurring psychiatric and substance use disorders (COPSD), and after hours crisis response and intervention. The ACT RN provides evidenced based services to assist individuals in maintaining or improving their ability to function in the community and work towards their recovery goals. Work hours may include evening and weekend hours to provide needed support and skills training to individuals and their natural support system.

Must be licensed as a Registered Nurse in the State of Texas.
- Must have at least one year of Mental Health Nursing experience.
- Must have valid Texas Driver License, a good driving record, and reliable transportation.

Must have at least one year of Case Management experience.
Two years of field based nursing is preferred.

Knowledge of community resources, of case management principles, objectives, standards, methods, and of program policies and procedures.

Be able to complete detailed, yet concise progress notes on all services provided during the sessions;
- Have the knowledge and experience in providing rehabilitative services to individuals with mental illness.
- Ability to assess individual needs, to coordinate services, to communicate effectively and to train others.

Have the knowledge of the social services system and the ability to connect clients with appropriate services;

Ability to write clearly and professionally;

Have the ability to think clearly and logically, in order to deal with potentially dangerous and/or emotional situations.

Working knowledge of Behavioral Health medications (antipsychotics, antidepressants, psychotropic agents, and mood stabilizers, and anticonvulsants)
