General:
· Functions as part of the interdisciplinary team to provide inpatient/outpatient palliative care/hospice and pain management services which may include:
o Recommends, monitors, and adjusts medications related to palliative care, hospice and pain management.
o Recommends, monitors, and adjusts medications for symptoms management of advanced illness and end of life.
o Obtains pertinent medical history and obtains accurate pain medication related history from patient via chart review.
· Orders, monitors, and interprets toxicology screens and other needed labs, implements follow-up on treatment actions as supported by lab interpretations.
· Participates in family and team meetings regarding end-of-life discussions.
· Performs rating scales on pain control, symptoms, and functional status.
· Provides patient and/or caregiver instructions and education.
· Uses available population health tools to identify and address risk mitigation needs focused on safe pain care.
· Communicates with team for next steps and coordination of care when necessary.
· Performs medication reconciliation in alignment with established policies and procedures.
· In addition to palliative care, hospice and pain management, orders and interprets labs and other needed tests and makes referrals.
· Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate recommendations for alternatives to current treatment plans as needed.
· Monitors and reports drug errors, adverse drug reactions, allergies, and patient compliance issues.
· Provides formal and informal education and in-services to pharmacy, medical, nursing, and other staff as requested.
· Provides drug information and drug literature evaluations as requested by the professional staff.
· Prepares and presents educational lectures to a variety of disciplines and patients as required. Provides education to patients, their families and caregivers about medications, advanced illness, and information on appropriate symptom management.
· Participates in self-development activities to maintain and strengthen professional competence in pain management and palliative care/hospice.
· Act in compliance with HIPAA rules and regulations.
· Work as directed by Supervisor.
