Description:
This is an part-time non-exempt position with the Pikes Peak Area Council of Governments Area Agency on Aging (PPACG AAA). The position is a Care Planner for informal caregivers who contact the Center for assistance. As a part-time care planner this individual is responsible for providing information and assistance to informal caregivers to help them access needed services in the community, support the Caregiver Respite Voucher Program and provide caregiver education to family caregivers and members of the community.
Duties include but are not limited to:

Provide care consultations for caregivers who contact the Center for caregiving information and services

Return phone calls from caregivers who call the Family Caregiver Center seeking support and assistance

Assist caregivers who walk into the Area Agency on Aging office seeking support and a consultation

Refer individual caregivers to the proper services and assist them with information on how to access those services

Assist the Family Caregiver Center Care Planner Supervisor with issuing monthly caregiver respite vouchers

Participate in community senior events involving the Family Caregiver Center, as needed

Serve as a back-up for the Family Caregiver Center Care Planner Supervisor when the supervisor is out of the office for an extended period of time

Assist with the planning and implementation of special community-wide caregiver events including the annual Caregiver Pampering Day

Assist with the scheduling and leading of the Powerful Tools for Caregivers course and the Caring Conversations Support Group

Assist with the development and provision of seminars and workshops related to caregiving to community groups, organizations, faith communities and businesses

Assist with the development and management of other special training and programs that address unique needs of family caregivers and that expand the program services of the Family Caregiver Support Center

Perform other work as requested by the Family Caregiver Center Care Planner Supervisor

Requirements:
Required Education and Experience

Master’s Degree in a human services curriculum, or
-
Bachelor’s Degree in a human services curriculum plus a minimum of 2 years of progressively responsible experience with an agency serving human resource needs, providing counseling or case management

Associate’s Degree in a human services field plus a minimum of 4 years of responsible experience with an agency serving human service needs, providing counseling or case management
-
Knowledge of community resources for the elderly and for caregivers of the elderly preferred, as well as of community service agencies

Good speaking and writing skills

Basic computer skills including Microsoft Word and Excel; database experience is helpful
