Job Description: Wound Care Nurse Practitioner
Position Summary: The Wound Care Nurse Practitioner (NP) works as a member of the medical team at EvoHealthMD to provide specialized patient wound in the comfort of their own home. This necessary role involves managing chronic, non-healing wounds, particularly those at stage 2, 3, and 4, using innovative treatments such as Amniotic skin grafts. The primary focus is optimizing wound healing outcomes and empowering patients to thrive in life.
Key Responsibilities - Wound Assessment and Management:
- Assess and document the extent of wounds, including stage classification.
- Develop and implement personalized wound care plans in collaboration with patients’ primary care physicians and/or home healthcare agencies.
- Administer wound therapies, including the application of Amniotic skin grafts.
- Monitor wound progress and adjust treatment as needed.
Patient Visits and Travel:
- Travel to patients’ residences, assisted living facilities, senior living communities, and nursing homes.
- Conduct thorough wound assessments during in-person visits.
- Provide compassionate care and education to patients and their families.
Charting and Documentation:
- Chart patient encounters promptly, either remotely or at your convenience, ensuring completion within 24 hours after the visit.
- Utilize Practice Fusion EHR system for accurate and comprehensive documentation.
Compensation and Benefits:
- This position operates on a pay-per-visit basis.
- An annual performance bonus is awarded based on individual and team achievements.
Qualifications:
- Board-certified Nurse Practitioner with a focus on primary care or family practice.
- Wound care experience is highly desirable but not mandatory.
- Strong communication skills and the ability to work independently.
- Commitment to providing excellent patient-centered care.
