UCP is looking for a fulltime LPN in Hyde Park, MA
Hours: 40 hours a week, Monday to Friday 8:30am to 4:30pm
United Cerebral Palsy of MetroBoston(UCP) is a progressive, non-profit agency committed to advancing the independence, productivity and full citizenship of people with disabilities. UCP specializes in working with a medically fragile population with varying cognitive levels. Each program provides therapeutic, recreational and community activities.
Qualifications: Current Licensed Practical nurse in Massachusetts with preferred experience working with individuals with developmental disabilities and physical disabilities. Must have a valid driver’s license.
Responsibilities: The responsibilities of the LPN include overseeing all aspects of health and medical needs of the individuals in the program, but not limited to:
- Administer medications, prepare/cut lunches according to swallow protocols, tube feedings, breathing treatments, wound care assessments, as well as monitor vitals and health of all individuals within the program.
- Conduct weekly health groups, provide comprehensive nursing assessments, and monthly medical record reviews for all individuals, as well as maintain written correspondence with providers, families, etc.
- Work closely and be supervised with the 2 RNs on coordination of nursing needs between both day program sites as needed, oversight of nursing documentation, development of medical policies and MassHealth regulation oversight
- Work closely with RNs and the Interdisciplinary team illustrating teamwork and hands on training to all members of the day habilitation team, including on staff OT’s & PT’s.
- Liaison to all families, caregivers and outside providers regarding health related concerns, having direct contact with families, caregivers, physicians, and outside vendors.
Working Conditions: The LPN is expected to commit the time and talents necessary to fulfill the requirements of the position in an exemplary manner. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Travel is primarily local and will be required to transport individuals in large or small vehicles. The LPN must possess the physical ability and willingness to assist persons served in personal care, which will include physical transfers and assistance with bathroom routines. Must have a full range of motion including the ability to bend, push, pull, bear-weight, and stoop for approximately forty-five minutes at a time.
