Position Summary
Under the supervision of the Clinical Director, the Case Manager will assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet the client’s needs. The Case Manager works collaboratively with the clients to provide services that meet their individualized needs and assists them in meeting their goals.
Essential Duties and Responsibilities
- Establishes specific goals and the actions and/or interventions that will be implemented to achieve client’s stated goals.
- Be the first face to greet the clients in the day.
- Reports, measures, and manages client success to ensure results and adequate progress is being made. Continuously monitors results to achieve long-term objectives.
- Communicates any pertinent information about the clients to the treatment team.
- Provides crisis management and ongoing assessment of client functioning.
- Documents all case management notes and case management treatment plans for clients.
- Provides appropriate referrals when necessary
- Facilitates as many weekly groups and individual sessions as required by the clinical director and the milieu.
- Attends team Case Conference meetings on a weekly basis to ensure coordinated implementation of treatment plans and service utilization such that the comprehensive care of clients reflects the philosophy and objectives of the facility
- Help client work towards GRIT (Client Volunteer track)
- Facilitate Neurofeedback (with training)
Qualifications
- Ability to use decision-making skills to solve complex problems with innovation and creativity.
- Demonstrates good interpersonal skills, promotes teamwork, manages conflict, and ensures a high degree of internal and external satisfaction.
- Demonstrates the ability to work independently, complete tasks in a timely manner, and maintain strong organizational skills.
- Ability to work with a variety of computer software programs.
- Knowledge of Mental Health
- Knowledge of Trauma Treatment modalities or the willingness and self motivation to learn.
- Knowledge of addiction, commonly abused substances and their classification, codependency, dual diagnosis and psychiatric conditions as well as understanding of treatment modalities.
- Knowledge of current laws, rules, and regulations within the industry.
- Knowledge of HIPAA Guidelines and Patient Privacy.
- Proficient in crisis intervention
- Maintains confidentiality of client Protected Health Information (PHI) used for treatment, payment, health care, and other personal information of all admits.
- Expresses compassion and understanding for the client’s situation and demonstrates care, which is appropriate for the age, diagnosis, cultural, religious, gender identity, or ethnic background.
- Knowledge of Brainpaint/Neurofeedback a plus
Required Education and Experience
- High school diploma or equivalent.
- Minimum 2 years in a Mental Health setting
- Minimum of 1 year in a behavioral health care/recovery setting.
- Associates, RadT and or Drug and counseling certification from an accrediting body or equivalent.
