We are currently seeking an experienced Social Worker with a “hospice heart” to join our team of Liaisons. This is a full-time position with flexible shift depending on patient availability. Some After hours and weekends might be required.

The Admission Coordinator/Liaison will coordinate hospice services after patients have been referred to the program, to assist in the transition from hospital care to Hospice home care; inform patients and families regarding the benefit of hospice care and insure continuity of care between hospital, home, skilled facility, physician and hospice RN; provide supportive counseling to patients and their families to cope with illness, transition to hospice and/or impending death.
Qualifications
- Master’s degree in Social Work required
- MSW job experience (minimum 1-year hospice EOB) required
- Hospice experience required
- Bi-lingual (English/Spanish) a plus !
- Valid CPR/Basic Life Support training required
- Valid driver’s license, car insurance and a good driving record required
- Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of patient’s status/care management.
- Participates in the development, implementation and evaluation of integrated clinical pathways.
- Provides supportive counseling and crisis intervention for patient/family as needed.
- Monitors patient’s progress to ensure care is appropriate and timely.
- Assists the Health Care Team in the coordination of care on the unit by serving as a resource to staff and assisting in day to day psycho-socially based problem resolution.
- Provides counseling and assistance with Advance Care Directives.
- Advocates for the patient to ensure quality outcomes.
- Coordinates and facilitates family meetings.
- Respects the dignity and confidentiality of the patient and all verbal/written information/communication.
- Conducts Education/Support Groups for patients/families as assigned.
- Provides emotional support to patient/family to facilitate adaptation to illness/disability.

Pay: $35.00 - $45.00 per hour
