ACDS, Inc., a respected Non-Profit in Plainview, Long Island, New York is in need of Community Habilitation (Com Hab) Specialists. As a Community Habilitation staff you will provide one-on-one support to people with intellectual and developmental disabilities. You play an integral role in teaching various life skills while providing recreational opportunities to promote community integration. The goal of this program is to empower the people we support to obtain greater independence
The Community Habilitation Specialist is responsible for providing personal care and skill development training for individuals with developmental disabilities in a manner that maximizes personal choice, rights, dignity and community inclusion. We have part time and full-time positions available in Nassau County and Suffolk County on both the North Shore and the South Shore. Day, evening and weekend shifts are available.
Requirements for the Community Habilitation Specialist include but are not limited to:
· High school diploma or GED.
· Experience working with individuals who have developmental disabilities.
· Direct Care experience preferred.
· Must maintain valid NY State driver’s license and maintain a clean driving record.
Key responsibilities include but are not limited to:
· Provide one-to-one support as needed in a variety of settings (home, community).
· Be a participating member of the individual’s team (circle of support).
· Ensure the health and safety of the individual by maintaining proper supervision and support at home, during transport and out in the community.
· Demonstrate a willingness to creatively work on the valued outcomes that are important to the individual which may include self-care, independent living skills, recreation, job readiness and community integration.
· Transport individuals to activities, classes, and outings in their community.
· Provide a positive model for the individual when out in the community.
· Complete all OPWDD mandatory training.
· Complete accurate and timely documentation of services provided.

Pay: $18.00 - $25.00 per hour
