Everest Healthcare Solutions is a growing Healthcare company that has various healthcare branches with exciting opportunities. Our home health branch which provides comprehensive care for medically fragile children is seeking a full-time Clinical Nurse Manager. The Nurse Manager will be overseeing patient-care operations, assigning and monitoring staff nurses, and identifying, and implementing quality improvements. Overall responsibilities are to provide assessment and oversight of care planning for Private Duty clients; assisting the Medical Director with program supervision, coordination, and oversight; providing resolution of client and caregiver matters. In this role the supervisor will be supervising field nurses and ensures that all preventive, therapeutic, restorative, and intermittent skilled nursing services are compliant.
DUTIES AND RESPONSIBILITIES
1) Conducting initial and re-assessments of clients in personl. Mentoring and development of field staff. Addressing/supporting clinical concerns. Coordinating visits by the Clinical Coordinator to observe clinical proficiency while with a client. Attending discharge meetings at the hospitals and extended care to generate a patient-specific plan of care.
2) Ensure Nurses and Home Health aides comply with observation, assessment, nursing diagnosis, counsel, care and health teaching, and the maintenance of health and prevention of illness of our patients.
3) Ensure that the clinical record for each patient receiving care is properly maintained.
4) Provide progress notes to the patient's physician about patients under care when the patient's conditions change or there are deviations from the plan of care or at least every 60 days.
5) Ensuring the home health aide assignments, care plan, and prepare written instructions for the aide, and supervise the aide in the home.
6) Coordinate the activities of the licensed practical nurse.
7) Act as the coordinator of the health care team in order to maintain the proper linkages within a continuum of care.
Develops an individualized plan of care in consultation with the client's physician and other appropriate agency personnel. Completes review of medications; assesses pain management and need for health care equipment/technology. Provides intermittent skilled nursing services, as ordered by the physician.
9) Conducts or participates in team meetings/case conferences to evaluate the client's response to care, alters the care plan, and makes referrals to other community agencies.
10) Training and teaching nurses on Gastrostomy tubes and care, Tracheostomy tubes and care, Emergency care, Emergency bags, individualized care plans and much more. Great teaching opportunity in the community.
Qualifications:
Associate’s or bachelor’s degree in nursing from an accredited college or university
Current Arizona license in good standing
Strong assessment and organizational skills
Demonstrated successful track record of working independently while taking on increased or diverse responsibilities
Current CPR Certification
