PART TIME working 26 - 29 hours per week
GENERAL SUMMARY:
Collaborate with the Hurley Food FARMacy Program Coordinator/Nutrition Specialist to provide patient-centered care to Hurley Food FARMacy clients and their families and regularly engage in program quality improvement projects.
SUPERVISION RECEIVED:
Reports directly to the Food FARMacy Coordinator/Nutrition Specialist.
RESPONSIBILITIES AND DUTIES:
- Maintain temperature logs on refrigerator, freezer, and other equipment and coordinates any maintenance issues and emergencies with hospital staff
- Ensure safe food handling procedures are in place and followed
- Data entry into electronic health records to track patient visits
- Manage data/reports of utilization
- Triage patients into appropriate HMC and community resources as needed
- Supervise volunteers/interns
- Communicate with referral partners to ensure program compliance
- Study and understand the financial needs of the program
- Research grants opportunities from government and non-government agencies
- Draft grant proposals and supporting documents
- Submit proposals to grant coordinators for approval
- Assist with development of culturally-relevant nutrition education materials
- Perform other duties as directed.
MINIMUM ENTRANCE REQUIREMENTS:
- High School diploma or equivalent.
- Undergraduate degree preferred.
- Ability to communicate effectively both orally and in writing.
- Ability to maintain accurate records using employer’s computer software.
- Knowledge of grants and grant application/eligibility processes
- Familiarity with the social determinants of health (SDOH) and patient-centered care
- Passion for sustainable food systems and health equity
Job Type: Part-time
Pay: $19.00 per hour
