POSITION TITLE: Care Coordinator Supervisor
REPORTS TO: Director of Agile Operations
CLASSIFICATION: Exempt
SUMMARY OF ESSENTIAL FUNCTIONS:
Responsible for the day-to-day management of the Patient Care Coordinator team across all Agile Physical Therapy locations (~10 direct reports), in coordination with the Operations and Clinical Directors.
ESSENTIAL JOB FUNCTIONS:
- Creating, implementing and auditing standardized work flows to create efficiency and effectiveness in the patient journey
- Stepping in where needed to assist in PCC daily activities/roles in a timely manner
- Responsible for working collaboratively with Operations and Clinical Director to ensure clinical success and an excellent patient experience
- This is a cross functional position that requires the candidate to take initiative and meet/align with necessary stakeholders to ensure efficiency at the PCC level
- Responsive and timely communication and follow up to ensure processes are moving forward
- Responsible to meet and report to the Operations Director weekly, or more frequently if necessary
- Developing, delivering, and promoting optimal standardization of the patient care coordination (PCC) team at Agile Physical Therapy, Inc.
- Recruit, interview, hiring, onboarding, & training new PCC candidates in conjunction with the Operations Manager and Director
- Implementing retention mechanisms to maintain highly skilled and motivated staff
- Overseeing and developing appropriate topics for PCC meetings
- Implementing and iterating on current or new workflows
- Identifying gaps in care or knowledge to be filled
- Creating an accountable, teamwork environment
- Creating timely accountability channels and outcome metrics for workflows to ensure consistent and excellent compliance with PCC staff
- Compile and report weekly key metrics on team and individual performance
- Performing annual reviews with the Operations Director when deemed necessary
- Performing monthly 1:1s with PCC staff to provide personalized support, discuss and document relevant metrics, address any concerns, and ensure the standardization and continuous improvement of patient care coordination
- Act as liaison between PCC team and billing department to help answer and resolve any patient billing issues or questions
- Effectively responding to and assisting with incident reporting to appropriate HR branch
- Explore and utilize technology where appropriate to streamline extraneous workflows
- Create professional and timely proposals for cost-sensitive new initiatives and workflows
- Comply with the Facility’s Comprehensive Compliance Plan (CCP) which includes but is not limited to: Business & Clinical Operations, Environmental Health & Safety, HIPAA, Human Resources and Code of Conduct as well as all facility policies and procedures and educational requirements associated the CCP
QUALIFICATIONS:
- Bachelor's Degree or above preferred.
- Healthcare operations or front desk experience required.
- Management and/or leadership experience preferred.

PHYSICAL DEMAND CAPACITY:

- Pushing---routinely loads of 5-150 pounds on casters, runners or pulleys
- Sustaining a grip---routinely for 1-2 minutes of 40-50 pounds
- Guarding a “lift load” routinely of 120-140 pounds
- Viewing monitor screen/using keyboard occasionally <4 hour duration
EXPOSURE DETERMINATION:
- Has the potential for Hazardous Substance Exposure due to work duties
- Has no potential for Bloodborne Pathogen Exposure due to work duties
HIPAA STATUS:
- Has been determined as eligible for access to/use of PHI & EPHI based on work duties and responsibilities in compliance with ‘minimal necessary’ standards

SALARY
- 85K-105K depending on experience and education
