JOB SUMMARY:
Oversees the provision of chronic case management, care coordination and clinical outreach team by assuming responsibility for personnel, management, and coordination of activities for Franklin Primary Health Center, Inc. (FPHC). Participates in the planning, organizing and implementation of activities that will increase patient users, encounters and community awareness of services offered at FPHC. Ensures that case managers/ care coordinators, perform as part of the multidisciplinary clinical team. Ensures that the case managers/ care coordinators facilitate and coordinate all aspects of the client’s access to needed services and resources. Maintains consistent communication and meets regularly with all third party partners to ensure that Quality and performance goals are met. Works cohesively with all departments and managers to promote Franklin programs and services.
RESPONSIBILITIES:
1. Participates with other members of the medical team in the development, implementation and evaluation of programs and Center policies.
2. Coordinates case management activities to support programs and services of the Center and cooperates in educational and training activities.
3. Develops/implements and evaluates monthly and periodic initiatives to increase the number of users as well as the number of encounters of all FPHC sites and locations.
4. Provide linkages to social and medical community resources. Maintains a list of community resources and acts as a point of contact for Franklin patient care teams as they search for access to community resources.
5. Responsible for the hiring, assignment, management, supervision, and evaluations of case management (medical and social service case managers) and clinical outreach staff. Ensures that outreach activities involving provision of medical services is fully staffed inclusive of afterhours and on weekends.
6. Establishes policies and procedures for case management and care coordination activities;
7. Develops, Plans and Implements activities to impact emergency room utilization rates, avoidable hospitalization s and readmissions (individual patient level and organization level)
8. Oversees the development of individualized case management plans as required for chronic care patients.
9. Develops budgets for case management activities and monitors spending closely to ensure that approved budgets are maintained and/or appropriate approval has been obtained for unexpected expenses.
10. Assists with in-service programs, community presentations, and health education activities. Maintains Franklin’s State approved CEU certification.
11.Keeps abreast of the environmental changes related to chronic case management and ensures that staff is educated on the appropriate chronic care management techniques as well as appropriate documentation and code submissions required in the EHR system.
12. Monitors and ensures that all required reporting is completed and submitted to the applicable party in a timely manner.
13. Tracks and reports case management, care coordination and outreach encounters for UDS.
14. Other assignments may be made at the discretion of the COO, CMO or the CEO.

QUALIFICATIONS:
Graduate of an accredited school of nursing leading to certification as Registered Nurse or Masters in Social Work with LCSW or LGSW. Candidate must have at least 2 years clinical experience and a minimum of three years supervisory or managerial experience. Two years of Case Manager/Care Coordination experience is preferred. Professionally licensed in the state of Alabama. Must have a valid Alabama Driver License.
KNOWLEDGE, SKILLS AND ABILITIES: Demonstrated knowledge of case management practices and skills, program development, referral sources. Knowledge of clinical procedures and guidelines. Skills in establishing and maintaining effective working relationships with patients, medical staff, third party payers, and the public are required. Skills are required in preparing and maintaining records, writing reports, and responding to correspondence. Ability to instruct, delegate and supervise a diverse team. Must possess strong communication skills; verbal, oral and written. Flexibility to undertake a variety of assignments. Ability to organize time efficiently among a variety of duties. Ability to present ideas to community and professional groups. Must possess the ability to communicate clearly and maintain confidentiality. Ability to prioritize and organize case load.
CERTIFICATION, LICENSES, REGISTRATIONS: Must have a valid driver’s license and a safe driving record and be able to pass a pre-employment physical and a criminal history background check.
CERTIFICATIONS, LICENSURES AND REGISTRATIONS: Registered Nurse or Alabama Social Worker's license, valid driver’s license, auto liability insurance.
