We have a LPN/RN position open and there is a $1000 sign on bonus!
- 16 hours (PT) 8 am to 4:30 pm OR 11am - 7:30pm. You would work every other weekend with on call rotation and work alternating holidays.
THE JOB:
The LPN/RN responsibilities include, but are not limited to; SAMM training, Skills Checkoffs for RCAs, In-Service facilitation, weekly medication audits, assessments of prospects and creation of service plans; communication with all resident support services, physicians, health care agencies, and families; quality assurance; and regulatory compliance.
- The LPN/RN is responsible for monitoring the health and wellness of our residents, in respect to overseeing activities of daily living, medication supervision, and assessing safety concerns of the residents.
- This LPN/RN will primarily work with our Dementia residents as well as work with our other residents as needed.
THE ESSENTIALS:
- You are encouraged to be fully vaccinated and boosted (if eligible) with COVID-19 and the flu.
- A Massachusetts LPN or RN License.
- 1-2 years experience with geriatric/dementia population.
- At least 2 years’ experience supervising CNAs or Home Health Aides (HHAs).
- Excellent organizational skills, clinical assessment and documentation skills.
- Knowledge of community support services.
- Experience in Assisted Living Facility required.
- Staff Development experience a plus.
THE DAILY:
- Documents and monitors the medication management program (SAMM) and observation of medications to residents by RCAs (CNAs/HHAs). Provides training for RCAs in demonstrating competence in Self Administered Medication Management Program (SAMM).
- The LPN/RN will perform introductory visits with new residents and family.
- Trains and supervises staff on SAMM and coordinates with pharmacist and other suppliers/vendors for medication delivery and accuracy.
- The LPN/RN documents and monitors health records and medical information, dementia evaluation history and current records reflecting resident’s physical and mental health.
- The LPN/RN works in collaboration with the Health & Wellness Director in the planning of the monthly RCA schedule.
- Performs weekly medication audits to ensure quality control. Reports medication issues to Health & Wellness Director and works with RCAs to ensure compliance in medication observation and documentation.
- The LPN/RN will work with the Health & Wellness Director in the coordination of elopement drills, emergency response plan and emergency service for residents, assisting with training staff on all shifts on company policy and response to emergencies.
- Triages emergency situations, and coordinates the delivery of medical or nursing care with care partners (PACE, SCO).
- The LPN/RN will act as a liaison to families, physicians and health agencies regarding changes and status in resident’s health and medication issues.
- Actively participates in care planning with the Health & Wellness Director. Directs RCAs to implement changes and achieve goals of the care plan.
- Maintains inventory of medical supplies, monitors expiration dates, and orders supplies as needed.
- In conjunction with Health & Wellness Director, the LPN/RN will be responsible for documentation of initial health assessment and care plans, as well as monthly progress notes and reassessment forms.
- Ability to work outside of regularly scheduled hours. Advance notice is not always possible, but will be given when it is.
- The LPN/RN must be on rotation for evening/overnight on-call.
- Complete incident reports and follow-ups as needed (incident reports within 24 hours of incident)
- The LPN/RN will maintain all records in compliance with the regulations of the Executive Office of Elder Affairs (EOEA)
- Other program/agency related duties as assigned by the Health & Wellness Director.
