Care Partner Support - Virtual Care Unit REMOTE
Responsible for serving as an extension of the Recovery Care Coordinator (RCC) and Social Services Coordinator (SSC) in patient care management. This role carries out the plan of care created by the RCC/SSC. Responsible for reinforcing the patient's confidence in self-management of their care and ensure that the necessary transition of care tasks are completed before the patient leaves Contessa Services.
- Part Time - 28 hours per week with potential for more
- Weekday hours will be Friday 7a-1p CST
- Weekend - Saturday & Sunday 7A to 6P CST
- Compensation is $21.00 to $24.00/per hour
Responsibilities
Supports RCC and Social Services as part of the recovery care team during monitoring phase.
- Coaches patient in Self-Management and arranges for closure of Gaps in Care.
- Contacts patient per protocol and educates on action plan developed for health improvement or behavior changes around self-care.
- Responds to specific alerts and escalates per protocol (Market ACC for on call hours) VCU during business hours.
- Transitions care tasks to ensure smooth transition back to PCP for ongoing care management.
- Provides administrative support for tracking ongoing care activities (vendor liaison, equipment pickup coordination, scheduling appointments).
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other related duties as assigned.
Qualifications
Required:
- High school diploma or equivalent.
- Willing to learn and practice motivational interviewing during patient interactions
- Willing to learn self-management model for patient support by non-licensed community health workers
- Ability to work weekends, 4 - ten-hour days and rotating holidays. This role works directly with market RCCs in assisting with care coordination - must be flexible based on patient needs and escalations.
Preferred:
- EMT-B certification.
- Some college experience.
- Medical professional such as medical assistant, certified nurse's assistant, health administrative assistant, or health coach.
