Patient Care Advocate
HealthCare Support is actively seeking a
Patient Care Advocate to fill an opening with a
fortune 30 company in
New Jersey.
Daily Responsibilities for Patient Care Advocate:
- Conduct member and Provider outreach (virtual, phone or in person)
- Education on care gaps and assist with scheduling appointments and/ or transportation
- Will be visiting providers throughout the state of NJ as needed, will WFH on the days not in the field
- Shift: m/f 9-5.
- Location: Hybrid Role, Working Remote in Southern New Jersey with Field visits to providers
Required Qualifications for Patient Care Advocate
- Education Requirement: Bachelor degree
- Years of Experience within Industry: 2+ years of experience in HEDIS and/or Quality Improvement - looking for experience coaching providers on how to improve HEDIS scores
- Technical Skills: has relationships with providers in NJ, experience assisting providers with improving patient outcomes/HEDIS measurements is highly preferred

Pay Details: $$25 - $$34/hour
