Responsibilities:
- Provide individual and family counseling for patients and families
- Conduct psychosocial assessment
- Formulate and implement the intervention plan
- Provide crisis intervention for patients who are in crisis
- Provide intensive case management/care coordination service for patients with complex bio-psychosocial needs
- Provide advocacy and referral service for patients
- Supervise interns and volunteers
- Complete documentations and reports
- Attend case conferences, agency meetings and staff meetings
- Conduct group/workshop for patients and the community
- Participate in community outreach and marketing activities
- Perform other duties as assigned
Minimum Qualifications:
- Master's Degree in Social Work from an accredited social work school

New York State LMSW/LCSW License required

Basic CPR (BCLS) Certification required (Health Center will provide if needed)

Working experience in social services field preferred

Familiar with the Asian-American communities

Good verbal and written English skills

Bilingual in English/Chinese required

Salary: $65,000.00 - 68,000.00 per year
