Essential Job Functions
· Ensure compliance to regulations for all referrals;
· Participate in onboarding appointments for accepted participants.
· Manage assigned case load of accepted participants;
· Facilitate Case Management meetings at least monthly;
· Provide assistance with obtaining/maintaining any Federal, State, and local benefits and provision of SOAR benefits applications;
· Educate participants on their housing rights;
· Provide life skills training as needed in order to maintain permanent housing including money management and home maintenance;
· Transport residents to view apartments; advocate on resident’s behalf with prospective landlords; must use own personal vehicle when agency vehicle is unavailable;
· Provide or refer to support services designed to stabilize households in permanent, habitable housing;
· Facilitate program graduations.
· Coordinate all aspects of utility and rental arrears payments, rental assistance payments, and other eligible financial assistance.
· Complete 3-month income eligibility reviews for assigned case load.
· Research permanent housing opportunities and maintain housing resource database.
· Coordinate all aspects of participants movement to alternative housing;
· Complete habitability inspections;
· Coordinate lease signings;
· Execute Security Deposits and Repayment Agreements.
· Manage outstanding Security Deposit repayments including monthly invoicing and incoming payments.
· Provide aftercare services to graduates consisting of contact with participants at least monthly for six months following graduation from the program.
· Required to be ambulatory and present and able to perform all job functions without limitations, up to and including life preservation activities.
· Responsible to comply with all Agency Policies including Agency Code of Conduct and Confidentiality Policies, Medicaid Compliance Program and Protocols and Justice Center Code of Conduct.
· Maintain attendance and punctuality. Notify supervisor when late or absent. Provide adequate notice when scheduling time off.
Job Duties include but are not limited to:
· Orient, train and supervise assigned staff including preparation of staff evaluations and interviewing prospective candidates for staff positions.
· Attend and participate in staff meetings, training sessions and other relevant meetings as scheduled.
·Develop and maintain relationships with local realtors and landlords.
· Participate in HMIS reporting and SOAR certification.
· Report to Support Services Supervisor.

Pay: $50,700.00 per year
