Located in the Upper East Side, Search and Care seeks out older adults in the community who need help managing life’s daily activities or accessing essential services. Search and Care provides clients with support and companionship to help them remain independent and safe in their homes. Search and Care currently serves adults over 60 in the Manhattan neighborhoods of Yorkville, Upper East Side, East Harlem, and the Upper West Side.
Position Overview:
The Case Manager has an awareness of the biological, psychological, and social impact that disabilities, illnesses and aging can have on the population that Search and Care serves. Under the supervision of the Clinical Director, the Case Manager will conduct assessments and will provide assistance with benefits/entitlements applications, referrals, and development of safety and service plans with clients according to NYC Aging guidelines. The Case Manager will oversee an assigned caseload of older adult clients and will remain in consistent contact with them to ensure they remain safely independent with an acceptable quality of life.
Specific responsibilities inherent to the position include:
- Provide comprehensive case management for a caseload of NYC Aging older adult clients.
- Perform home visits for new and existing clients for intake, assessment, and reassessment.
- Ability to provide services in office, in the field, and online/telephonically.
- Demonstrate knowledge of benefits and entitlements and ability to connect clients to those benefits.
- Provide referrals, advocacy, and care coordination to assist clients with linking to other service providers as needed and appropriate.
- Assess clients for potential referral to other in-house services
- Provide timely documentation and maintenance of client files and progress notes in agency database and NYC Aging program according to NYC Aging standards.
- Display a team-oriented approach and a strong ability to collaborate effectively with agency colleagues and outside community service providers, including social services agencies, hospitals/doctors, and local elected offices.
Qualifications/Skills:
- BS in related field (MSW and/or LMSW strongly preferred).
- Experience working with NYC Aging funded programs preferred.
- Strong knowledge of benefits/entitlements and community resources available to older adults preferred.
- Willing and able to conduct home visits regularly.
- Writing, time management, and documentation skills.
- Proficiency with Outlook 360, NYC Aging Peer Place, and other standard programs preferred.
- Team player who can work collaboratively and independently.
- Fluency in Spanish is preferred.
- Complete all other responsibilities and duties as assigned by the Clinical Director
