Position: Case Manager
Location: Harrisburg, PA
Hours: Monday-Friday; 10am-6pm

The Case Manager is responsible for providing case management services to individuals and families ages 18-24 as they transition out of homelessness into permanent housing. The Case Manager serves as a resource broker who possesses a strong understanding of local resources in order to connect residents with services and supports needed to maintain permanent housing. The Case Manager will help individuals and families identify permanent housing options, submit applications, assist in transitioning into their new home, and develop life skills needed for independent living in the community. This position requires a person who is familiar with the needs and challenges of this population and is committed to helping them successfully transition out of homelessness. The Case Manager is expected to exercise considerable initiative and independent judgment in achieving objectives while performing in accordance with established regulations, policies, and procedures.
Applicants must be able to demonstrate a high proficiency in case management skills, an understanding of human services systems, and heightened attention to detail. Must be highly organized, able to multi-task, set priorities, and have excellent written and oral communication skills. A high degree of proficiency with Microsoft office software is required.
Essential Duties and Responsibilities
- Assist participants in identifying and securing permanent housing.
- Coordinate move-in activities.
- Role model mature pro-social behavior.
- Provide case management services to residents assigned by program staff.
- Conduct intake sessions, needs assessments, and individual service and/or housing plans with each resident on caseload.
- Maintain weekly contact with residents and document accordingly.
- Complete progress and case notes for all resident-related activities within 24 hours of service delivery.
- Work with residents to develop life skills necessary to achieve self-sufficiency.
- Make referrals to support service agencies within the community as appropriate and support residents in navigating the process.
- Coordinate with agency and community resources to ensure residents have access to services, resources, and aftercare support needed to meet their personal goals.
- Arrange access to public assistance, social security, food stamps and medical assistance appointments to ensure timely and continued receipt of benefits.
- Assist residents in scheduling and attending medical and dental appointments, mental health and substance abuse services, and obtaining medication refills, when appropriate.
- Assist residents in pursuing vocational and educational programs.
- Assist residents in developing a discharge/aftercare plan that includes services and supports needed to live successfully in the community.
- Collect program evaluation information from residents on caseload.
- Ensure confidentiality of all resident information.
- Participate in all staff development, training, and meetings as required.
- Follow all policies and procedures of the organization.
- Ensure compliance with all federal, state, local and funding source guidelines.
Qualifications
- Associate Degree in Social Service field (preferred) and/or a minimum of two years case management experience in the Human Service field
- Proficient in Microsoft Office
- Must possess a valid driver’s license, active auto insurance, and vehicle
- Criminal background and ChildLine clearances required
Job Type: Full-time
Pay: $18.00 - $22.00 per hour
