Summary
Under the supervision of the QMPH, THE Mobile Crisis Peer Support Engagement specialist provides peer led support and is responsible for fostering a safe, motivating and supportive environment.
Role and Responsibilities
· Welcome new guests upon arrival
· Provide an explain of the program to guests as to what they can expect
· Establishing professional relationships with guest
· Consult with consumer run programs such as self-help groups and consumer drop-in centers
· Facilitate client participation in self-help programs and activities
· Provide referrals for additional services
· Foster open communication with guests and serve as an advocate for guest
· Must maintain long term stable recovery
· Demonstrate competency in personal recovery and use of coping skills
· Utilize the Wellness Recovery Action Plan (WRAP) and other resources
· Assist guest to plan and carry out leisure time activities
· Accompany guest to social and community activities
· Develop supportive services and tools to address barriers
· Develop activities to include recovery education, information, resources and connection to natural supports
· Follow principles of trauma informed care
· Create and distribute communication for guest
· Provide side-by-side support and encouragement to help guest socialize
· Provide self-help recovery strategies
· Provide guidance for locating and utilizing other community resources
· Organize and lead individuals and group social and recreational activities to help guests structure their time, increase social experiences, and provide opportunities to practice social skills
· Must Complete all required training in a timely manner
· Must meet the following performance standards:
100% of individuals contacted within one business day
100% of individuals engaged during hospital stay
100% of Individuals engaged on day of discharge
100% of Individuals engaged at 30 days post discharge
100% of individuals who require assistance with Medicaid application receive it
100% of registrations submitted according to DMH policy
Qualifications and Education Requirements
· High School diploma or equivalent preferred
· Must have CRSS credential or complete 100 hours of CRSS training within one year of date of hire
· Effective oral and written communication skills
· Must be service oriented, empathetic and able to interact and develop rapport with a diversified population
· Current CPR and CPI preferred
Must pass background checks and pre-employment drug screen High School diploma or equivalent preferred
