Job Summary:
We are seeking a dedicated and compassionate Hospice DPCS to join our team. In this leadership role, the Hospice DPCS will be responsible for overseeing the delivery of high-quality care to patients in hospice settings. This position requires strong clinical expertise, excellent communication skills, and the ability to effectively manage a team.
Duties:
- Provide leadership and guidance to the interdisciplinary hospice team
- Ensure compliance with all regulatory requirements and standards of care
- Admit patients to hospice services
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to develop individualized care plans
- Conduct regular assessments of patient needs and adjust care plans as necessary
- Monitor the quality of care provided by staff members and provide feedback to support improvement
- Coordinate with community resources to ensure continuity of care for patients transitioning from hospital or nursing home settings to hospice care
- Participate in performance improvement initiatives to enhance patient outcomes
- Educate and support patients, families, and caregivers regarding end-of-life care options
- Provide on-call support as needed to support the clinical team:
Experience:
- A minimum of 2 years of experience in hospice or palliative care required
- Demonstrated strong knowledge of nursing home regulations and standards of care
- Preferred experience in home care or community-based settings
- Experience with HospiceMD preferred
Skills:
- Excellent leadership and management skills
- Strong clinical assessment and critical thinking abilities
- Effective communication and interpersonal skills
- Ability to collaborate with a multidisciplinary team
- Proficiency in electronic medical record systems
