FULL JOB DESCRIPTION
SALARY: $25.00 - $27.00 PER HOUR, $52,000.00 - $56,160.00 ANNUALLY
JOB TYPE: FULL-TIME

Our open position at CSI is for an Intellectual and Developmental Disabilities (IDD) Case Manager, working on our Washington County contract. On our Washington County contract, we have a team of nine IDD case managers total, and this position is our only current opening.
As an IDD Case Manager, you will provide individualized support to children and adults with disabilities with a focus on person-centered thinking, self-determination, empowerment, and respect with a commitment to adaptability in this ever-changing world. As an IDD Case Manager, you will:
- Manage a caseload of approximately 50-55 individuals
- Meet face-to-face with the people you support a minimum of twice per year. These meetings can take place at day programs, residences, hospitals, schools, etc.
- Help assess needs, plan support to meet those needs and enhance independence, coordinate funding for the support services, make referrals to service providers, create service agreements to supply payment to the service providers, and ensure that all service providers are continuing to meet the needs of the individual on an on-going basis
- Write annual plans (CSSP) that describe and demonstrate the hopes, dreams, and outcomes of individuals and clearly describe areas of need and support
- Completing case notes in SSIS, which also serve as documented time for billing
- Assessing need for crisis services, and completing referrals as needed
REQUIREMENTS
- Bachelor’s degree in social work and LSW, OR a bachelor’s/4-year degree in a related field such as Sociology, Psychology, Human Services, etc.
- Experience working in human/social services
- A valid driver’s license, a reliable vehicle, and proof of auto insurance on that vehicle
- Background clearance
- Excellent organizational skills and ability to track timelines
NICE TO HAVE
- Experience with Rule 185, DD waiver, MN Choices, Medical Assistance, and Medicare
- Computer competency and experience with Microsoft 365
