Title: Case Manager

This position has an introductory probationary period of 90 days.
THE POSITION: The Case Manager (CM) will closely support a caseload of vulnerable individuals experiencing houselessness by eliminating barriers to sustainable housing. Duties will include assisting participants with goal identification and completion, helping them complete necessary housing and service documentation/applications, connecting them to resources that improve their overall wellness, and supporting them with community integration. This position will be a part of a program team that engages participants in improving their overall health and well-being using strengths-based, housing first, and harm reductive models. Case Managers working in shelter sites are additionally expected to assist with shared site operation duties such as meal distribution, cleaning tasks, and other duties as assigned.
THE PROGRAM: Arbor Lodge is a congregate shelter located at the corner of Denver and Lombard in North Portland. The program will serve up to 106 participants at a time, including 18 outdoor village-style pods, 80 indoor congregate-style beds. Arbor Lodge will also include a state of the art kitchen and a professional chef. The site is open 24/7, 365 days a year, and therefore all program staff are considered essential workers.
HIPAA REQUIREMENTS: This position will have access to Protected Health Information (PHI) during the course of their work activities. Applying the minimum necessary standard of HIPAA, the designated record sets to which this position may have access include all sections of the medical record, patient demographic information in the practice management system, etc. This position is required to read the content of these records only to the extent needed to accomplish the assigned tasks.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Engage with your assigned caseload and advocate for particpants' needs by assisting them with identifying their individual goals to obtain and thrive in stable housing.
- Conduct weekly case management meetings with all participants on your caseload. Follow procedures outlined by Do Good and the Joint Office of Homeless Services (JOHS) to clearly documenting all engagement with participants, and their ongoing health and well-being.
- Maintain accurate, up-to-date physical and electronic records utilizing the Homeless Management Information System (HMIS) and CaseWorthy. Enter and exit participants from the database, track services and progress evaluations, maintain ROIs, and perform regular reporting functions.
- Develop comprehensive knowledge of community and Veteran resources and maintain effective relationships with these resources to best meet participant needs.
- Assist participants in applying for eligible benefits, including Oregon Health Plan, food stamps, job training programs, etc.
- Maintain consistent, clear, and respectful communication with all staff, relevant Management and Director team members, programs, and community partners.
- Attend staff supervisions, staff meetings, and any appropriate community professional meetings.
- Attend all required trainings as instructed by the Program Manager. Advance relevant professional knowledge/skills with the guidance of the Program Manager to drive continuous improvement in the program’s effectiveness.
- Participate in compliance, quality assurance, and quality improvement activities as directed.
- Assist in delivery of hygiene kits, food boxes, and other essential supplies to individuals your assigned caseload.
- Follow program protocols to maintain a safe environment for staff and participants.
- Adhere to all state and federal privacy and security regulations applicable to the program, and to Do Good Multnomah’s policies and agreements regarding confidentiality, privacy, and security.
- Perform other duties as assigned.
SKILLS & ABILITIES:
- Knowledge of procedures, techniques, and standards to optimize individual case management practice.
- Demonstrated ability to work with individuals with serious and persistent mental illness (SPMI) and/or substance use disorder.
- Consistent, clear/direct, and compassionate communication with participants, staff, referrals, community partners, and vendors. Ability to communicate clearly and concisely both verbally and in writing.
- Demonstrated ability to work independently, problem solve, and complete tasks in a timely manner. Ability to organize multiple high-priority tasks in a fast-paced environment.
- Ability to maintain accurate, confidential records and information in an organized manner.
- Ability to display a non-judgmental and non-biased attitude at all levels of the organization.

- Ability to use motivational interview techniques, strength-based advocacy, and trauma-informed care.
- Ability to maintain appropriate professional boundaries with both participants and staff, including utilizing discernment to know what is appropriate to share and what is confidential.
