*WAYNE COUNTY, EDWARDS COUNTY, WABASH COUNTY, HAMILTON COUNTY AND WHITESIDE COUNTY*
EDUCATION AND WORK EXPERIENCE REQUIREMENTS:
Preferably a BA/BS degree in social science/social work, or related field.
One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, replace one year of college education up to and including four years of experience replacing a bachelor degree.
Under the direction of the Office Supervisors, Assistant Directors and the Director of Case Coordination Unit Services the Case Manager is responsible for completing in home assessments making participants eligible for the Community Care Program through the State of Illinois, Illinois Department on Aging. In addition, they complete assessments, case plan development, conduct quarterly home visits, follow up on authorization of service, referrals, and maintenance of case records.
FUNCTIONS:
- Administration of the appropriate intake form and appropriate web site checks accordance to funding sources rules and regulations.
- Conduct the Choices for Care Assessment in the hospital, Long Term Care Facility and home.
- Complete Initial and Redetermination Assessments in their home for individuals over the age of 60 and link them to the appropriate services.
- Interim or Temporary Service Increase Assessments relating to home and community-based services. Making appropriate referrals and responding to applicant/client request.
- Maintaining case records, including documentation of assessment and all referrals and follow-up.
- Conduct a 5 month and 9-month home visits to review the current Person-Centered Plan of Care and the required documentation and communication with respective Case Manager.
- Maintain effective working relationships with provider agencies, hospitals and long-term care facilities.
- Documented provision of training on Illinois Department on Aging and Area Agency on Aging policies, procedures, and case management techniques.
- Attendance at training with a minimum of 24 hour per calendar year.
- Report Elder Abuse and Neglect and service problem/issues to appropriate agencies.
Home Visits, Face to Face assessments, Field position.
Travel as assigned.
Other duties as assigned.
