We are seeking a committed Licensed Vocational Nurse (LVN) for our Subacute Department to join our AWESOME TEAM!

- Position Type: Full Time - 4x2 Schedule; 5 days a week; 8 hrs per day
- Shift Schedule: AM Shift (7:00 AM to 3:00 PM) or PM Shift (3:00 PM to 11:00 PM) or NOC Shift (11:00 PM to 7:00 AM)
- Pay Range: $38 to $42 per hour

RESPONSIBILITIES:
- Administer medications and treatments as prescribed by physicians
- Monitor vital signs and report any changes or concerns to the appropriate healthcare professionals
- Assist with activities of daily living, including bathing, dressing, and feeding
- Provide emotional support and reassurance to residents and their families
- Document and maintain accurate patient records daily
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Adhere to all facility policies and procedures, including infection control and safety protocols
REQUIREMENTS:
- Valid and active Licensed Vocational Nurse (LVN) license in the state of California.
- Must have an active BLS through the American Heart Association
- Must have an active CPR Certification
- Must have a High School Diploma or GED
- Is in good standing with all regulatory agencies and licensing boards is required
- Interest in working with the geriatric population
- Completion of an accredited vocational nursing program.
- Knowledge of the nursing process and standards of care for skilled nursing facilities
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Excellent critical thinking and problem-solving abilities
- Basic computer skills and proficiency in electronic health records
