JOB DESCRIPTION:
Holliday’s Helping Hands (HHH) which is an organizations that care for homeless families and individuals - We support the cause and fight to end homelessness! Our jobs focus on providing interim housing, nourishing meals, and safe locations to empower our clients to find their “Journey to Home.”
Under normal supervision, Case Managers provide strong clinical and case management leadership to ensure client receives high-quality care that exceeds DHS/ODR standards for services. This position is responsible for maintaining the highest quality provision of direct individual and group therapeutic and case management services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs a combination, but not necessarily all, of the following duties:

§ Provide individual and group counseling services, and intensive case management and outreach based on the needs of the clients

§ In collaboration with clients and the HHH team, evaluate needs of clients and development treatment/service plans accordingly

§ Develop effective, trusting relationships with clients, with a focus on facilitating independence, empowerment, and improved physical and mental health
-
§ Assist in involving clients in daily activities and scheduled appointments, budgeting, facilitating socialization, health and housing maintenance, community integration, and achieving life recovery

§ Develop and maintain complete, accurate, and current client records in electronic databases and client charts as required by HHH and DHS standards

§ Provide case updates as requested by supervisor and in HHH team meetings

§ Provide input and evaluation of programming as a participant of HHH’s quality
improvement program

§ Ensure that mental health services are reflective of HHH model which emphasizes
customer choice, harm reduction, and psycho-social rehabilitation
-
§ Cooperatively and cohesively work with other clients of the multidisciplinary team and
HHH departments, including active participation in department and agency staff meetings
and trainings

§ Collaborate in the development and provision of clinical trainings

§ Complete Adult Initial Assessment, Discharges, and Progress Notes on time and within
industry, DHS, ODR, and HHH standards of practice without exception

§ Meet productivity goal of a minimum of 65% of work time in direct service provision

§ Assist in providing clinical information and support to the HHH treatment team

§ Ensure that all notes are up to date in the HHH system and chart within 48 hours of
delivering services

§ Follow all company and government policies relevant to HHH operations

§ Work proactively and cooperatively with co-workers, staff, and management
-
§ Attend staff meetings, trainings, huddles, and other meetings as requested by HHH

§ Other duties as assigned.

Education & Experience

§ High school diploma and AA in the social service field required; Bachelor Degree in relevant field desired;

§ Three years’ experience working with marginalized populations;

§ Timely and accurate charting skills;

§ Previous experience in healthcare preferred but not required; and
-
§ Conflict management and/or de-escalation experience required;
