Job Summary:
In this role, you will provide monitoring and chronic care management support to patients, ensuring they receive timely and effective care. This position offers the flexibility of working remotely while being an essential part of our patient care team.
Key Responsibilities:
- Monitor patients' health data remotely using advanced telehealth technology.
- Assess and interpret patients' vital signs and other health indicators.
- Provide timely interventions and follow-up care based on monitoring data.
- Supports and facilitates patient compliance to RPM, chronic care management (CCM), and provider’s plan of care
- Communicate effectively with patients, families, and healthcare providers to ensure continuous care, we work to close the loop with the primary care provider and ensure compliance.
- Work closely with clinic providers/staff members to manage day to day calls with enrolled patients for screenings, medication adherence, patient and family education, health maintenance reminders, medication refills, required interventions, and other services as needed
- Document all patient interactions and interventions accurately in Care Cove’s software platform.
- Participate in multidisciplinary team meetings to discuss patient care plans and progress.
- Identify and address any barriers to the patient’s primary care physician’s plan of care and compliance.
Qualifications:
· Graduate from an accredited school of nursing
· Current license to practice as an RN or LVN/LPN in Arizona and/or Nursing Compact States
· Minimum of 2 years' experience working with adult and senior populations, preferably with care management experience in a primary care practice setting.
· Ability to work independently and manage time effectively in a remote environment.
· Proficiency with electronic health records (EHR).
· Reliable internet connection and a dedicated HIPPA-compliant workspace.
