POSITION OVERVIEW:
The Preventive Case Manager is responsible for working with the clinical teams to provide education, counseling and support services to patients/families affected by substance use disorders. This includes screening, referrals, crisis intervention, individual and group counseling, case management, and care coordination of substance and mental health counseling services. Settlement Health is committed to the awareness, prevention and treatment of opioid abuse and of other controlled substances for patients with a high rate of comorbid mental health and substance use disorders requiring a comprehensive, integrated treatment approach, with concurrent diagnosis and treatment, for support of health and improvement of quality of life.
ESSENTUAL DUTIES AND RESPONSIBILITIES:
·Provide adolescents and parents with education about early identification and prevention of substance abuse disorders.
· Oversee, manage and continually re-assess the established program that identifies patients receiving controlled substance prescriptions (including opioids, benzodiazepines, etc.)
· Provide support to clinicians in their management of patients who are current users of chronic opioids and/or other controlled substances. This includes the completion of controlled substance agreement, periodic pill counts and/or follow up on Utox screens as ordered by the clinician.
· Evaluate patients with chronic opioid use for the presence of co-occurring mental health conditions and refer for treatment as appropriate.
· Evaluate patients on chronic controlled substance prescriptions for abuse and/or misuse of these medication or other drugs and refer to the appropriate level of care in collaboration with the provider.
· Provides Screening, Brief Intervention, and Referral to Treatment (SBIRT)
· Implements additional questionnaires (such as AUDIT and DAST), in order to identify at risk patients with the goal of reducing and preventing related health consequences.
· Provide interventions for patients identified by the providers as struggling with Alcohol Use Disorder by providing psychoeducation and referrals to the appropriate level of care.
· Expand evidence-based mental health and substance abuse prevention and education programs for patients, families, communities, and personnel to increase awareness of, patient access to, and patient retention in mental health and substance use disorder treatment programs.
· As part of a comprehensive approach to reducing risk for people struggling with opioid abuse, increase awareness of the appropriate use of naloxone to reverse opioid overdoes through patient, family, community, and personnel training.
· Empower patients with mental health and substance use disorders to make informed decisions about their care, including pain management alternatives, treatment options, and recovery through peer counseling, patient education, or other evidence-based strategies.
· Enhance education for patients, families, communities, and personnel to support patient engagement and self-management that includes medical conditions that often co-occur with mental health and substance use disorders (e.g., diabetes mellitus, heart failure, hepatitis, HIV/AIDS, hypertension, obesity).
· Provide short term pre-treatment counseling to patients whom are ambivalent about their need for treatment and help them move towards motivation for change. This can include Individual or Group modalities.
· Consultations with primary care practitioners to provide assessments, information and referral as needed.
· Act as a liaison to health care providers, community agencies and other staff to ensure patients access to services within SH as well as the community, and to address any barriers for the patient in accessing care.
· Participate in community outreach and educational activities to promote Settlement Health and program’s services.
· Participate in regular supervisory meetings; attend Departmental Staff meetings and other meetings as assigned.
· Maintain, Track and Monitor all Outreach, referrals (in-house and to community), services (Clinical / Concrete), along with any other expectations of the Grant.
· Immediately report any problems or unusual occurrences to supervisor.
· Participate in weekly crisis coverage providing crisis assessment, intervention, and safety planning when providers deem it necessary.
· Collaborate with Behavioral Health team members to support patient assessment, treatment, and referrals for substance use as needed.
· Attend supervision and staff meetings.
· Other special projects/duties as assigned.
REQUIRED QUALIFICATIONS:
- Master’s degree in Counseling, Psychology, Social work or related field.
- Credentialed Alcoholism and Substance Abuse Counselor certification (CASAC)
- English & Spanish fluency, required.
- Available to work on Saturdays as part of care management team rotation or as needed, required.
- (5) five years’ experience providing addiction service prevention, education and counseling services.
- Experience working with patients in primary care setting preferred.
- Experience with Electronic Health Records preferred.
