General Functions: Support residents to achieve their Outcomes as they strive for greater independence living and working in the community. Create and oversee individuals’ Implementation Strategies, ensuring progress toward meeting individual Outcomes. Advocate for service innovations and operational changes consistent with the needs of the individuals who receive services. Specific Functions:
- Conduct annual staffing's in order to create Implementation Strategies for each resident by using an interdisciplinary approach. Arrange for special staffings with the client, other staff, family and/or guardians, and others as needed. Amend strategies, as indicated.Provide training and guidance regarding services and supports as identified in each resident's Implementation Strategy, following up with Managers and other personnel to ensure that the services are being provided and documented.Complete monthly QIDP notes for each Outcome, actively working with the individual and their team members to amend any Outcome that is exceeding or underperforming according to the individual’s preferences.Maintain Electronic Case Record and paper case files in a timely and accurate manner, scanning/uploading, and filing to each. Perform periodic quality control audits for client records, as assigned.Complete and/or monitor completion of all annual assessments (ICAP, SLOF, etc.). Ensure all medical services are being provided by the assigned entity ( i.e., annual physicals, dental exams, routine and follow up appointments).Participate in all Psychiatric Reviews and Psychiatric Evaluations, ensuring meaningful documentation is completed ahead of time.Act as liaison for all Emergency Room visits and any ensuing hospitalizations, ensuring consistent, thorough, and meaningful documentation of each is provided. This includes responding to all urgent situations 24/7 in order to provide guidance and support to all parties involved.Maintain all client benefits (Medicaid, Medicare, SNAP/LINK, Housing Authority) in order to ensure that no lapse in coverage occurs. Actively work to maximize all benefits to the fullest extent possible.Maintain awareness of client's health and activity levels, assuring that each client promptly receives needed medical treatment from community providers as available resources allow.Maintain an active liaison with family members or significant others regarding the client's engagement with programming, progress, etc.Communicate on a regular basis with other program staff to assure consistency in supports and services.Utilize the agency's online work order system to notify the Buildings and Grounds Department of necessary repairs or other structural needs in homes.Provide counseling and crisis intervention to clients in need of such assistance and guidance.Be familiar and act in accordance with established safety regulations, policies, and procedures.Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather and other hazards.Maintain excellent on-time attendance.Perform other unspecified tasks and duties as assigned.Maintain ongoing compliance with all agency-related training and certification requirements. Achieve 12 CEUs per fiscal year.Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.Perform other unspecified tasks and duties as assigned.

Qualifications: Bachelor's degree in human services and appropriate experience with demonstrated ability preferred. Non-degreed candidates with at least five years of relevant experience in mental health/human services may be considered. Driving record and auto insurance in accordance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE.
