Full job description
Case manager is responsible for planning the patients' visits and schedules, patient needs, and clinician availability. The employee must maintain excellent relationships with the field clinicians, doctors' offices, be aware of their availability and special skills, analyze patients' cases and their needs. The employee must submit the schedules before the end of the day and report to the DON.
Responsibilities
- Accept new referrals on daily basis and create schedules based on the referral requirements.
- Organize active patient schedules based on the priorities and requirements.- Manage, organize, and troubleshoot the patient and staffing log every day.
- Keep field staff availability updated and posted.
- Keep the EMR system up to date.
- Maintain excellent relationships with the clinicians.
- Communicate with the nurses and patients and troubleshoot based on the situations.
- Provide accurate information and updates to nurses for daily schedule.
- Create the route of the nurses properly.
- Performs other duties as assigned.
