Duties:
- Provide direct patient care, including administering medications, performing procedures, and monitoring patient vital signs.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
- Offer health coaching to patients, providing guidance on healthy lifestyle choices and disease prevention.
- Manage durable medical equipment (DME) for patients in need of assistive devices.
- Utilize electronic health record systems such as Alora Plus to document patient information accurately.
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice.
- Associate's degree in nursing or equivalent education and experience.
- Prior experience in Home Health is a plus.
- Strong knowledge of healthcare practices, procedures, and regulations.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
