Qualifications/Educational Requirements:
- Valid Physical Therapy License
- Accepting New Graduates
- Ability to relate positively and favorably with patients and staff
- Demonstrate good oral and written communication along with good documentation skills
- Must be able to travel within different areas of the Bay Area including, but not limited to, San Francisco and San Mateo County
- Must have valid driver's license and current auto insurance
Responsibilities/Essential Functions:
Patient Assessment & Care Development:
- Conduct thorough assessments of patients, considering their medical history, physical condition, and referral requirements.
- Develop individualized care plans aligning with patient needs and goals, following physician's orders and evidence-based practice.
Supervision of Physical Therapist Assistants (PTA):
- Provide direct supervision and support to PTA, ensuring adherence to the plan of care.
- Foster ongoing communication, provide feedback, and make necessary adjustments to ensure effective treatment.
In-Home Therapy Interventions:
- Deliver skilled therapy interventions and treatments in a home setting employing a variety of therapeutic techniques as appropriate.
- Monitor patient status, progress, and adherence to the care plan, making adjustments as necessary.
- Educate patients and caregivers on managing conditions and achieving goals, and prepare them for discharge when appropriate.
Clinical Documentation:
- Complete clinical notes, evaluations, and progress reports within the agency's established timelines, maintaining adherence to both legal and professional standards.
- Ensure that all necessary forms and documentation are accurate, clear, and reflective of the care provided.
Care Coordination & Conferences:
- Attend and actively participate in care conferences with other healthcare professionals, ensuring an integrated approach to patient care.
- Collaborate with the care team to achieve a seamless transition between different stages of therapy and other services if needed.
Quality Improvement Participation:
- Actively participate in the agency’s quality improvement program, suggesting improvements where necessary.
- Continuously strive for excellence by adhering to best practices, attending training, and maintaining up-to-date knowledge of physical therapy trends.
Additional Responsibilities:
- Perform other related and assigned duties that may include liaising with other healthcare providers, participating in community outreach programs, or assisting with administrative tasks.
Qualification Requirements:
The Physical Therapist in this position must hold the appropriate state licensure as a physical therapist and demonstrate the ability to perform the above essential job functions with or without reasonable accommodations.
