CHHA/CAREGIVER Camden Gloucester and Burlington Countys
POSITION SUMMARY
Responsible for providing personal assistance services to the client in order to maintain safety in the client?s home. The CHHA/Caregiver reports to the Director of Nursing and/or Nursing Supervisor for all clinical requirements and matters, and the Care Manager for staffing and administrative matters.
QUALIFICATIONS
Must be 18 years of age or older
Must be able to speak, read and write in English
Must have honesty and integrity
Must hold a current valid certificate in good standing issued by the New Jersey Board of Nursing
Must not have a criminal record which would bar employment
Must be able to tolerate a physical workload including standing or sitting for prolonged periods of time
Must be able to tolerate a variety of housing conditions
DUTIES AND RESPONSIBILITIES
Satisfy all clinical competency review requirements
Perform personal assistance services for clients as dictated by client needs/preferences, examples of which include:
Assist in personal care including, but not limited to, lifting, transferring, turning, dressing, grooming, walking assistance, bathing, changing and toileting
Clean all areas used by client including, but not limited to, sweeping, mopping, dusting, dishwashing, linen changing, laundry and trash removal
Prepare and administer client meals according to the client?s dietary needs and wants
Errand running for the client, including, but not limited to, delivering bill payments, mailing letters and grocery shopping
Transport client in the client?s automobile with proper release form and proof of insurance and current driver?s license. If client has no vehicle or it is not usable, may transport client in personal automobile or company owned vehicle with current auto insurance, driver?s license and MVR results are clear.
Other responsibilities include:
Consistently attends assignments as scheduled; consistently arrives on time; demonstrates flexibility in accepting assignments and staying late if needed.
Report any and all violations of client?s rights
Respect the client?s personal property and report any damage, destruction or removal
Complete and submit all documentation (Progress Notes/Tendio Visit Notes) as designated in a timely manner.
Maintain client confidentiality at all times
Maintain professional relationships with the clients and professional work habits
Communicate with immediate supervisor of changes in schedule or issues affecting ability to perform assigned duties
Communicate to Nursing Supervisor any changes in client?s condition
Participates in initial and mandatory annual in-services
I agree to contact this office within 48 hours after a job ends permanently to obtain further assignments, and to make myself available by telephone or in person for future assignments. Failure to do so may result in forfeited job assignments and that unemployment benefits may be denied. I understand that all work assigned is considered temporary and that no full time permanent work has been offered.
