Job Title: Qualified Mental Health Professional (QMHP)
Focuses on working closely with community hospitals, providers, and stakeholders to engage adults and/or children/adolescents in mental health/substance use services. This position provides proactive intervention and care management (treatment planning, assessment, referral/linkage, and monitoring) to ensure that members and recipients receive appropriate assessment, oversight, and services. This is a mobile position with work done in a variety of locations. Duties and responsibilities:
- Provide intensive in-home (IIH) or mental health skill building services (MHSB) in a one-to-one, family (if applicable) and group setting for individuals, with emphasis on parenting skills training, behavioral, social, employment, and/or recreational needs.
- Implement positive behavior support plans as required
- Develop individualized support plans (treatment plans) per case
- Models and teach problem solving skills
- Initiate referrals for other community based services and supports not offered by Accord
- Coordinate visits with school personnel to monitor the youth’s academic achievement as needed when appropriate
- Coordinate and provides transportation to services and medical appointments as needed when appropriate
- Coordinate scheduling of appointments with the individual and family (if applicable) on a weekly basis
- Provide 24-hour crisis intervention and support for the individual and family as needed
- Monitor for side effects of psychotropic medications and provide psychosocial education for the purpose of medication and recovery
- Document all service delivery in a timely manner as required by Department of Behavioral Health and Developmental Services
- Maintain regular and ongoing contact with referring agencies, as well as other agencies involved in delivering services to the individual and his family
- Actively coordinate and plan discharge and aftercare plans with referring agencies
- Attend interagency meetings and supervisions as necessary
- Attend and participate in team meetings
- Attend and participate in orientation, in- service training, and conferences
- Perform other duties as assigned.
Knowledge, Skills, and Abilities:
- Familiar with assessment tools
- Ability and knowledge to develop and implement service plans
- Knowledge of counseling and in-home procedures
- Effective written and verbal communication skills
- Strong interpersonal, communication and documentations skills
- Behavior management training and the ability to manage challenging behaviors and implement positive behavior support plans
- Available community based services and supports
- Effective delivery and presentation methods and styles
- Scheduling and time management
- Must be registered with BOC as QMHP- C, A, or T and obtain required CEU’s
