Provides and/or supervises skilled nursing care services to patients as directed by the physician and pursuant to objectives, policy and procedures of JUBILEE HOME HEALTH CARE, INC.
DUTIES AND RESPONSIBILITIES (List major functions in order or responsibility)
1. May participate with the hospital health care team members and the physician in evaluating the comprehensive health needs or patients in planning for discharge and/or planning for home health care services.
2. Promotes continuity of care from hospital to home by acting as liaison between patient and/or family physician, hospital and the Agency
3. Arranges for coordination and implementation of the discharge plan when it involves referral for home health care services.
4. As directed by the Physician, implements home health care services following the
policies, procedures for acceptance of patients.
5. Works independently with minimal direct supervision.
6. Makes the initial evaluation visit.
7. As directed by the physician develops and implements the patient care treatment plan. Provides a safe and sanitary environment for the patient; adheres to JUBILEE HOME HEAL TH CARE, INC. Infection Control and Safety Management policies.
8. Completes all documents necessary for implementation of services.
9. Provides and/or supervises nursing services, rehabilitative treatments, diagnostic rehabilitative and preventive procedures defined in the treatment plan.
10. Re-evaluates the patient's nursing needs on a regular basis by a visit to the patient's place of residence.
11. At least every sixty (60) days reports to the physician the patient's progress in relation to the treatment plan and assures that a new treatment plan is completed and signed in order to continue services.
12. Observes and reports to the physician any changes in signs, symptoms, reaction to treatments, drugs, services, physical and/or emotional responses that may indicate a need for change in the treatment plan.
13. As directed by the physician, makes appropriate changes in the patient's treatment plan.
14. Teaches supervises and/or counsels patients/family members in providing for self care/help activities in order to move them toward independence.
15. Supervises activities of other Agency nursing service personnel and documents appropriately.
16. Provides for and/or participates in orientation of new personnel and other educational activities.
17. Participates in and/or conducts team conferences.
18. Provides consultation to other professional disciplines as needed.
19. Charts daily record of patient care on clinical progress notes.
20. Completes other records.
21. Ensures a safe environment for patient care.
22. Implements care based on Plan of Care and JUBILEE HOME HEAL TH CARE, INC. policies.
23. Coordinates acquisition of necessary equipment.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT:
Work is of medium demand - walking, standing, driving, occasional lifting and turning of patients. Frequent reaching, handling, fingering of equipment for patient care.

EDUCATION, EXPERIENCE, TRAINING (Include Licenses and Profession Affiliations)
1.Be licensed to practice professional nursing in the State of California.
2. Current California Drivers License.
3. Preferably one-year home health experience.
4. Two years acute hospital experience required.
5. Has well-developed nursing assessment and interviewing skills.
6. Current CPR certification.
