Job Title: Caregiver Liaison and Scheduler
Location: Bethany Village, Mechanicsburg, Pennsylvania
Reports To: RN Clinical/PCSS Manager
Job Summary: Interim HealthCare is seeking a Caregiver Liaison and Scheduler to join our team at Bethany Village. This role will provide essential support under the guidance of the RN Clinical/PCSS Manager. The Coordinator will play a pivotal role in resident advocacy, coordination of care services, and supporting administrative operations. This position requires strong communication skills, a background in home care specific to the elderly, and proficiency in office support tasks. Time will be split between attending events and activities with residents on campus and performing office duties.
Key Responsibilities:
Resident Advocacy and Support:
- Serve as an advocate for elderly residents and their families, addressing social, emotional, and environmental needs through compassionate and effective communication.
- Provide support through daily interactions and engagement with residents and caregivers.
Coordination of Care Services:
- Coordinate and facilitate access to healthcare, social services, and community resources to meet the continuum of care needs of residents.
- Ensure continuity of care by communicating effectively with healthcare providers and community organizations.
- Conduct scheduling and daily conversations with existing and future caregivers.
Administrative Support:
- Assist in collecting and organizing documentation for the HR Department and other departments as needed, ensuring compliance with regulatory standards.
- Maintain accurate resident records and documentation using office software and systems, supporting efficient administrative operations.
Resident Engagement and Activities Coordination:
- Organize and participate in social and therapeutic activities tailored to residents’ interests and abilities to promote well-being and socialization.
- Attend events and activities with residents on campus, managing scheduling and logistics to ensure smooth coordination and positive experiences.
Collaboration and Team Support:
- Collaborate closely with the RN Clinical/PCSS Manager and interdisciplinary team members to support comprehensive care plans and optimize resident outcomes.
- Foster a supportive and inclusive team environment, contributing to the overall success of the care team.
Qualifications:
- Experience: Must have experience in home care specific to elderly residents, with a minimum of two years of relevant experience.
- Education: Associates degree in gerontology, social, medical field, or related areas preferred.
- Skills:
- Strong interpersonal and communication skills, with the ability to engage effectively with elderly residents, caregivers, and their families.
- Proficiency in office support tasks, including documentation management, scheduling, and administrative duties.
- Excellent organizational skills with meticulous attention to detail.
- Ability to utilize computer software for data entry, communication, and reporting.
- Compassionate, patient, and empathetic approach to resident care, committed to promoting dignity and quality of life.
Working Conditions:
- Primarily office-based role at Bethany Village, Mechanicsburg, Pennsylvania.
- Requires flexibility in scheduling to accommodate resident needs and participate in occasional evening or weekend activities.
Physical Requirements:
- Ability to walk many steps, help set up displays, and move chairs.
- Must be able to lift and carry items up to 50 pounds.
- Occasional standing, walking, bending, and reaching.
- Ability to use office equipment and computer systems for extended periods.
