Position: Home Health Occupational Therapist
We are seeking a dedicated individual for a full-time, part-time, or contract-based role as a home health Occupational Therapist. In this position, you'll serve as a crucial partner to our patients, helping them develop,recover or maintain daily living and work skills, particularly for those with physical, mental or developmental conditions. You will also provide effective and patient-centered services, empowering patients to confidently and independently carry out everyday tasks. Finally, the successful candidate will demonstrate a well-rounded perspective and possess the ability to assess and adapt the environment to meet each patient's needs.

Location/s: Union City, Hayward area (Our therapists choose which locations they can cover)

Responsibilities:
- Address clients' performance aspects to support engagement in activities or occupations that impacthealth, well-being, and life quality.
- Select and adapt appropriate occupational therapy treatment models, methods, and approaches on a per-case basis to direct interventions.
- Apply interventions to enhance safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
- Teach clients new strategies for approaching tasks and educate them on breaking down activities into achievable components.
- Assess clients' home and other environments, offering advice on alterations consistent with their needs and capabilities.
- Recommend adaptive equipment and provide training on its usage.
- Guide and educate family members and caregivers.
- Observe, note, and report progress toward long and short-term treatment goals.
- Collect data and document processes followed.
- Regularly follow up with clients.
Job Requirements:
- Minimum 1 Year OT Experience
- Preferably at least 1 year Home Health experience (Training provided for OT without home health experience)
- Must have valid professional license to practice OT in the State of California
- Valid CPR License
- Valid Driver's License and your own reliable transportation
