Position Summary
The HFA Case Manager (Advocate) provides in-person, family-centered, evidence-based health education, parenting education, child development education and activities. Case management services consist of completing assessments, facilitating referrals and any other beneficial supportive services.
Essential Functions:

Provide intensive case management services, including assessments and referrals, through in-person home visits in one of the following services areas: Berks County, Bucks County, Delaware County, Philadelphia County, Lehigh County or Montgomery County

Flexibility is key - be ready to adapt your schedule to meet family needs.

Assist families in identifying their health and social service goals.

Support parent-child engagement and foster family well-being through reflective strategies.

Collaborate with community resources to ensure comprehensive support for families.

Conduct health promotion activities and deliver evidence-based parenting education.
To be well-equipped and excel as an HFA Case Manager (Advocate) you will possess the below qualifications:

Associate's degree in Social Work, Public Health, or related field of study such as: Family Studies, Women’s/Gender Studies, Human Services and Education.

Experience with community health issues.

Valid driver's license with driving experience and a clean driving record.
