TEAM LEADER - COMMUNITY HABILITATION
Progressive human services provider seeks reliable and well-organized individuals who can multi-task and play an individual role in multifunction teams to serve as a Community Habilitation Team Leaders. Under the direction of supervisor, the CH Team Leader facilitates Person-Centered Planning and coordinates individualized services. Supervises the community habilitation staff (DSP), ensuring effective service delivery. Promotes independence and community inclusion of individuals served through creative planning, effective use of staff management and professional teamwork. Develops individualized plans with DSP and ensures compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies. Experience with Precision Care or other electronic case record software used in the Human Services Community.
Summary:
The work is performed under general supervision. The team leader plans and carries out assignments and handles problems and deviations in accordance with experienced judgment, instructions, policies, procedures, and/or accepted practices. Work is evaluated for competency of professional judgment and achievement of goals and objectives.
Eprine programs enable individuals to receive the care they need in their homes and communities and reduce the need for out-of-home placement. Using strength-based and family-focused approaches, Eprine develops programs that build on and improve the successes of the individuals we serve.
What We Require of You:
- Perform all work according to agency and program-specific protocols and productivity standards
- Provides an assessment of all new program participants.
- Monitor and track time-sensitive documents; Life Plan’s, SAP’s, LCED, etc.
- Maintain up to date files for program participants in Precision Care, ensuring timely documentation and updates in EHR according to established time frames.
- Ensures that the program remains in compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies.
- Other duties as assigned.
Minimum Qualification Requirements
- Bachelor’s Degree in the field of education, social work, psychology, vocational rehabilitation, guidance, and counseling or other related human services field from an accredited program, with at least two (2) years of experience supporting people with intellectual and developmental disabilities.
- One (1) year Leadership and Supervisory experience preferred.
- Knowledge of Home and Community Based waiver programs, DSP Core Competencies - and ability to keep up to date to such knowledge – required.
- Knowledge and understanding of Person-Centered Planning.
- Ability to establish and maintain a working relationship with participants, families, and staff.
- Ability to supervise and provide role modeling to staff effectively.
- Strong verbal and written communication skills necessary.
- Ability to organize work with self and others, as well as delegate work to others as appropriate.
- Field visits, on-call, flexible schedule, weekends, evenings, and some weekends required.
- $2,000 sign on bonus available.

Pay: $60,000.00 - $65,000.00 per year
