We are seeking an experienced MDS Director with at least five years of experience as a nursing home director to join our team. The MDS Director will be responsible for overseeing the completion and accuracy of the MDS assessments for all residents, ensuring compliance with state and federal regulations, and leading the interdisciplinary team in the development and implementation of individualized care plans.
Key Responsibilities:
- MDS Assessments:
- Oversee and ensure the accurate and timely completion of MDS assessments for all residents.
- Review MDS assessments for accuracy, completeness, and compliance with federal and state regulations.
- Ensure that assessments are accurately documented and submitted to CMS
- Interdisciplinary Team Leadership:
- Lead and coordinate the interdisciplinary team in developing, implementing, and updating individualized care plans based on MDS assessments.
- Facilitate regular care plan meetings with residents, families, and staff.
- Regulatory Compliance:
- Stay current with changes in MDS regulations and ensure the facility's compliance with federal and state requirements.
- Prepare for and participate in surveys and audits, ensuring all documentation is in order.
- Quality Improvement:
- Monitor and analyze MDS data to identify trends and areas for improvement.
- Develop and implement quality improvement initiatives based on MDS data findings.
- Staff Education and Training:
- Provide ongoing education and training to nursing and clinical staff on MDS processes, regulations, and best practices.
- Mentor and support staff to enhance their understanding and performance of MDS-related duties.
- Resident Care Coordination:
- Collaborate with nursing staff to ensure high-quality resident care that aligns with care plans.
- Address and resolve any issues or concerns related to resident care and MDS documentation.
- Documentation and Reporting:
- Maintain accurate and up-to-date documentation of all MDS assessments and care plans.
- Generate and review reports related to MDS data for internal and external stakeholders.
Qualifications:
- Education: Bachelor's Degree in Nursing (BSN)
- Licensure: Current Registered Nurse (RN) license in good standing.
- Experience: At least 5 years of experience as a nursing home director, with substantial experience in MDS coordination.
- Knowledge: Comprehensive understanding of MDS, federal and state regulations, and long-term care processes.

Pay: $120,000.00 - $150,000.00 per year
