POSITION OVERVIEW
The Case Manager is part of the Epiphany-FTC Project treatment team and is responsible for the providing comprehensive case management services, formulation and implementation of the individual program plan, coordinating all services and referrals provided to clients and maintaining up to date data and written documentation.
Starting Salary: $30/hr
QUALIFICATIONS
- Bachelor's degree in behavioral science, preferred.
- Certification as a drug and alcohol counselor or registered and in the process of certification.
- Minimum 4 years of experience working with at-risk women, children, and families.
SKILLS AND KNOWLEDGE REQUIRED
- Have effective written and verbal communication skills.
- Ability to work well within a multi-disciplinary team and follow trauma-informed principles.
- Ability to work effectively with families from diverse cultures in collaborative manner.
- Proficient in computer skills with various software as assigned.
- Possess valid CA Driver's License and a safe driving record for local travel only.
- Must meet agency conditions for employment including requirements for Child Abuse and Fingerprinting clearances.
- Flexible in working hours and days to accommodate program needs.
RESPONSIBILITIES
- Participates in daily meetings to be updated on client needs and develop team plans.
- Enters client data accurately into computer system as assigned.
- Meets with clients weekly to provide case management and treatment planning.
- Functions as the Epiphany Center liaison with the Department of Social Services workers, Public Health, and other community agencies as needed.
- Makes referrals to other agencies and services as determined by the Individual Treatment Plan.
- Facilitates Recovery and Life Skills groups as needed.
- Demonstrates and maintains appropriate boundaries with clients and other staff.
- Develops an after-care plan with each client and assists with transition into aftercare services.
- Maintains client confidentiality as required by professional ethics and legal requirements.
- Participates in weekly staff meetings and in-service trainings as required.
- Required to drive agency vehicles. Transport clients in agency vehicles as need to meet goals.
- Communicates effectively with coworkers, c1ients and personnel from outside agencies.
- Acknowledges safety policies and procedures, and report potentially unsafe conditions.
- Supports and implements the Epiphany Center mission and values.
- Other duties as assigned.
