Position: Case Manager/Recreation Assistant
Location: Quincy/Boston, MA
Pay rate: $21.71
Dinners and mileage will be reimbursed.
Hours: Part-time, 15-20 hours per week. Schedule may vary.
What you'll be responsible for:
The essential functions of the position are below. Additional duties may be assigned as required.
- Demonstrate ability to provide instruction, education, and training to adults with brain injury in their home and the local community with the goal of maintaining independence;
- Responsible to communicate with clients and their families as well as other providers, and community resources;
- Develop community-based resources and maintain effective communication;
- Provide support coordination/case management including intake, assessment, service planning and coordination, progress reporting, documentation, and client advocacy;
- Proficiency with scheduling, organization, and time management;
- Proficiency with Microsoft office;
- Assist Recreation Coordinator in running a variety of community-based activities/events which include, but are not limited to: trips to Castle Island, Webb Park, Boston Bowl and planning nights at area restaurants, shopping centers, local concerts, and the Movie Theater;
- Assist in supervising participants including, but not limited to, supervising small groups and individuals, providing 1:1 ratio if needed;
- Provide direct support & contact with individuals and caretakers;
- Take leadership responsibility in the event of an emergency. This may include calling emergency services or contacts, and assisting with transportation needs.
Requirements:
- Bachelor’s Degree or background in health and human services preferred

At least one year of experience supporting adults with Traumatic Brain Injuries

A passion and dedication to supporting our clients is a must

Valid Driver’s license and use of personal vehicle for work-related duties
