Overview: The CareOne LTAC’s (Long Term Acute Care Hospital) mission is to define excellence within the health care community. We work to maintain the highest standards of care and service for patients, families and our valued employees.
A Clinical Liaison serves as an advocate for patients during the pre-admission process to our Long Term Acute Care Hospitals. You serve as a resource for the hospital and healthcare professionals and develop results-producing relationships throughout the medical facilities.
As the premier ventilator weaning facilities in New Jersey, the CareOne Hospital Division LTACs offer a post-ICU plan of care that focuses on getting patients back to their baseline of health. Clinical Liaisons help hospitals in their local communities to identify post-ICU patients who would benefit from a longer stay in an ICU step down level of care.
