About the Job:
Our patient coordinator team sits at the heart of our operations, connecting physicians, pharmacies, health plans, and patients with the industry-leading service we’re known for. We’re seeking a patient coordinator to join our Nashville team to help build and coordinate our community, onboard interested patients, and educate healthcare professionals, so we can continue in our vision to transform the care experience for people at home.
You will be responsible for community engagement, visiting doctor offices, demoing our medication device, calling prospective patients, and coordinating the onboarding process for those who want to start with our service. This role offers a great combination of remote call work, with days on the road visiting doctors, pharmacy site visits, and dropping by community events. It’s all about building relationships with patients, doctors, the pharmacy team, and community programs. Communication, sales, and organizational skills are key to thriving in this role.
**While this is a remote position, we require applicants to reside in Nashville, Tennessee.
Key Responsibilities:-
Building and maintaining strong relationships with physician clinics around Tennessee.

Setting up appointments and demoing our medication adherence product.

Identifying community organizations that could benefit from our solutions and engaging with them to share our vision.

Calling consumers who have requested more info, or been referred, explain the service, verify coverage, and assist with onboarding.

Liaison between enrolled patients and assigned pharmacy partners.

Organize and track patient progress through the onboarding process.

Seek to develop and maintain strong customer relationships while identifying new business.

Manage CRM pipeline and schedule to maximize opportunities.

Ad hoc duties.
Requirements:-
Strong customer relationship skills.

Outgoing and personable. Not afraid to build relationships from scratch.

Highly organized and detail-oriented.

Driver’s license and vehicle.

Ability to work in a fast-paced growing environment.

Pharmacy, social work, or care aid experience is a plus.

Experience using a CRM is a plus.

Excellent verbal and written communication skills with professional telephone presence.

Superior organization and time management skills with the ability to multitask.

Proficient in computer use and software programs.
