Duties:
- Provide comprehensive social work services to patients in a medical setting
- Conduct assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate patient care
- Assist patients and families in navigating healthcare systems and accessing community resources
- Provide crisis intervention and support during times of emotional distress
- Conduct research and stay up-to-date on best practices in medical social work
- Specialize in areas such as autism, PTSD care, crisis management, or assisted living
- Advocate for patients' rights and ensure their needs are met
- Provide counseling and support to patients and families dealing with complex medical conditions
- Collaborate with other healthcare professionals to ensure holistic patient care
Skills:
- Strong communication and interpersonal skills
- Ability to assess and address the psychosocial needs of patients
- Knowledge of community resources and ability to connect patients with appropriate services
- Experience working with diverse populations, including children and older adults
- Crisis management skills and ability to provide immediate support during emergencies
- Familiarity with electronic medical records systems and documentation requirements
- Ability to work collaboratively in a multidisciplinary team setting
- Empathy, compassion, and sensitivity when working with patients facing challenging situations
