Home Health Setting
The Physical Therapist in a Home Health setting is responsible for the evaluation of patients in relation to home assessment, functional status and mobility, safety awareness, and other physical therapy needs based on medical necessities
The Physical Therapist is responsible for collaborating with the physician and medical care team in evaluating the patient's level of function, determining the treatment plan, providing skilled intervention as well as supervising personnel to ensure treatment goals are achieved to maximize and restore their patient's functional skills to the fullest in their home setting.
Must have a valid Illinois driver's license, vehicle insurance, and dependable automobile. Or must be able to commute easily to the patient’s location.
Must have a current CPR card.
