Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality medical care to patients in the comfort of their homes.
Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Perform medical procedures such as IV insertion and wound care
- Educate patients and their families on managing illnesses and injuries
- Coordinate with other healthcare professionals to ensure comprehensive patient care
- Provide skilled nursing services including medical-surgical care, pain management, and hospice palliative medicine
- Oversee durable medical equipment (DME) use and provide ostomy care
- Manage patient cases, including discharge planning and ongoing support
Requirements:
- Valid Registered Nurse license in the state of practice
- Proficiency in medical imaging techniques
- Experience in case management and discharge planning
- Ability to lift and move patients safely
- Skilled in IV insertion and pain management techniques
Nice-to-have Skills:
- Familiarity with hospice palliative medicine practices
- Previous experience in medical-surgical nursing
- Knowledge of durable medical equipment (DME) usage
