The Radiant Light is spreading & we are growing! Radiant Hospice seeks a compassionate Hospice LVN experienced with providing direct in-home care to patients approaching end of life. Hospice LVN completes patient visits as assigned and coordinates with the IDG team to ensure continuity of care. The Hospice LVN receives assignments from the Triage Manager and makes visits to the patient's place of residence and facilitates the hospice patient's care. The Hospice LVN will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. Hospice LVNs evaluate a patient's physical condition and provides care to address any of the patient's medical needs. Apart from providing comfort to patients, RNs are adept at counseling and guiding patient's families as well.
Schedule/Pay Structure as follows:
- Saturdays from 08:00 to 20:00: Standby Shift (Paid $150 Standby Fee per shift)
- $100 per completed patient visit (as assigned by the Triage Manager)

Minimum Qualifications:
- Graduate of an accredited nursing program.
- Current California Licensed Vocational Nurse license.
- At least one (1) year of hospice or palliative care experience
- Minimum of two (2) years of experience as a professional nurse within the last five (5) years.
- Expertise in pain management, symptom control, crisis intervention, and family support.
- Excellent demonstrated critical thinking skills and superior clinical judgment.
- Excellent computer skills.
- Excellent customer service skills.
- Reliable cell phone with local number and coverage.
- Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable personal transportation.
- Ability to work independently and manage time effectively. Ability to make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting.
- Acceptance of and adaptability to different social, racial, cultural, and religious modes.
