The primary responsibilities of the Intake Coordinator include:

Contact patients who have been referred or who have been self-referred within 24 business hours of the referral.

Determine appropriateness of the referral, review with a physician if unsure. Prioritize referrals by their urgency.

Monitor the electronic fax queue for referrals and patient records related to referrals. Initiate the referral process or add the records to the patient record within the EMR.

Ensure complete demographic, insurance and pertinent clinical information is received for the referral.

Schedule appropriate referral patients to be seen in clinic, work with the Director of Practice Operations regarding any schedule availability issues.

Reviews details and expectations about the referral with the patient.

Contact referring offices for follow-up if needed.

Work with the Physician Liaison regarding any referral workflow issues.

Collect patient records needed for referred patients following HIPAA.

Represent the practice professionally and appropriately with all referred patients, referring offices and internal stakeholders.

Document all patient interactions in the patients chart ensuring that it meets billing guidelines and expectations for services.

Collaborate with other departments and external care providers to address and resolve issues that impact patient care.

Participate on various committees and project groups related to area of focus within the practice.
Ideal candidates hold:

Minimum of 1 year in a community clinic setting.

Proven work experience in a role or environment that emphasized teamwork, strategy and community healthcare.

Associates Degree preferred but not required.
This position is part-time, approximately 20-25 hours per week.
