**Job Summary:**
As a Licensed Practical Nurse (LPN) at a Nursing Home, you will be an integral part of our healthcare team, providing direct nursing care to residents under the supervision of registered nurses or physicians. Your dedication to compassionate care and attention to detail will contribute to the well-being and comfort of our residents.
**Key Responsibilities:**
1. **Direct Patient Care:**
- Provide skilled nursing care to residents in accordance with established care plans and physician orders.
- Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
- Administer medications and treatments as prescribed, ensuring accuracy and compliance with policies and procedures.
2. **Vital Signs Monitoring:**
- Perform regular assessments of residents' vital signs, such as blood pressure, pulse, temperature, and respiratory rate.
- Monitor residents for signs of distress or changes in condition and report findings to the appropriate healthcare provider.
3. **Wound Care and Dressing Changes:**
- Assess and dress wounds, ulcers, and skin conditions according to established protocols and standards of practice.
- Monitor wound healing progress and document findings accurately in residents' medical records.
4. **Health Education and Support:**
- Educate residents and their families on topics such as medication management, disease management, and self-care techniques.
- Provide emotional support and reassurance to residents and families, promoting a positive and nurturing environment.
5. **Documentation and Reporting:**
- Maintain thorough and accurate documentation of nursing assessments, interventions, and resident responses.
- Report significant changes in residents' condition to the registered nurse or physician in a timely manner.
6. **Team Collaboration:**
- Collaborate effectively with interdisciplinary team members, including registered nurses, physicians, certified nursing assistants, and therapists.
- Participate in care planning meetings and contribute valuable insights to the development of individualized care plans.
**Qualifications:**
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Massachusetts.
- Completion of an accredited practical nursing program and passing the NCLEX-PN examination.
- Previous experience in a long-term care or geriatric setting is preferred but not required.
- Strong clinical skills, including medication administration, wound care, and vital signs monitoring.
- Excellent communication and interpersonal abilities, with a compassionate and patient-centered approach to care.
**Physical Requirements:**
- Ability to lift and move residents, as needed, with or without assistance.
- Stamina to stand, walk, and perform physical tasks for extended periods.
- Manual dexterity and visual acuity sufficient for performing nursing procedures and using medical equipment.
**Work Environment:**
- The position involves working within a nursing home setting, which may include exposure to infectious diseases, unpleasant odors, and challenging resident behaviors.
- Flexible scheduling may be required, including evenings, weekends, and holidays, to ensure 24/7 coverage and meet the needs of residents.
